Individual competitors must complete an SSFA Provincial 55+ Games Registration Form, a Registration Waiver Form, and a Health Form. All three forms are available below. Additionally, they are available on the SSFA website (middle of page), or a registration package can be obtained by contacting your district coordinator. The district coordinator contact information is available here. The registration form and waiver, upon completion, are then submitted to your district coordinator. Individuals and team members must purchase a SSFA membership from their district and pay the registration fee for the Provincial competition to their district office.
Note: The Competitor / Non-Competitor Health Form must be filled out for the games. Bring the form to the Games registration and this will be worn inside your name-tag for the duration of the 55+ Games. Additionally, the Code of Conduct sheet must be printed and worn inside your name-tag pouch as well.
